Volunteer Agreement for Youth Football Camp Coaching

By entering name and date and clicking SUBMIT button below I certify this action will constitute an electronic signature in lieu of an actual signature and evidences the parties’ mutual intent to enter into a valid and binding contract that will satisfy all applicable legal requirements for the Agreement, Sexual and Physical Abuse/Molestation Policy and Safety/Injury Control Procedures. These Agreements shall constitute an “original” when printed from electronic files or records established and maintained in the normal course of business.
This Volunteer Agreement ("Agreement") is made between the Coach, the High School, and in association with the NFL Alumni Association and Pro Sports Experience (PSE). The purpose of this Agreement is to outline the responsibilities and terms for Coach’s role as a volunteer for a non-contact youth football camp. Please complete below:
1. Position and Responsibilities: Coach agrees to volunteer as a High School Football Coach for the youth football camp jointly operated by the High School, NFL Alumni Association, and Pro Sports Experience. The Coach will: A) Provide football coaching to youth participants. B) Deliver instruction in a safe, non-contact manner according to camp guidelines. C) Promote positive sportsmanship and teamwork. D) Act as a role model, demonstrating respect, fairness, and integrity. E) Adhere to all safety protocols and guidelines established by the High School and Affiliates. F) Follow all safety protocols provided by the High School, NFL Alumni Association, and Pro Sports Experience. G) Report any incidents, injuries, or violations of camp rules to the camp organizers (On-Site Camp Football Director and PSE] promptly.
2. Duration of Service: Under the Direction of the on-site Camp Football Director, the Coach agrees to volunteer every scheduled hour of the camp’s operation, as specified in the camp schedule.
3. Background Check and Clearances: Coach agrees to complete any background checks or clearances required by the High School and/or Affiliates, following all relevant state laws and school district policies. All required clearances must be approved before the camp begins.
4. Liability Waiver: Coach acknowledges the inherent risks involved in participating as a volunteer at the camp. Coach agrees to release and hold harmless the High School, NFL Alumni Association, Pro Sports Experience, and their respective employees, officers, and agents from any liability for injury, accident, or harm related to volunteer activities.
5. Non-Compensation: Coach understands that this is a volunteer position and will receive no monetary compensation, benefits, or employment rights from the NFL Alumni Association, or Pro Sports Experience.
6. Termination of Agreement: This Agreement may be terminated at any time by either party, with or without cause. Notice must be provided in writing if either the Coach or High School chooses to end this volunteer arrangement.
7. Acknowledgment: By signing below, Coach acknowledges that they have read, understood, and agreed to the terms and conditions outlined in this Volunteer Agreement.

SEXUAL and PHYSICAL ABUSE/MOLESTATION POLICY
Pro Sports Experience is committed to providing a safe sport environment. This policy applies to all affiliates, partners, directors, officers, volunteers, coaches, independent contractors, athletes, managers, officials, employees, and members of Pro Sports Experience.
It applies to abuse which may occur during the course of all business, activities and events relating to PSE in any way. As an affiliate, partner, employee, independent contractor, coach, volunteer, or worker, you may NOT 1) Go anywhere alone with a child 2) Use corporal punishment against any child or adult 3) Engage in hazing or demeaning conduct against any child and/or adult.
When any affiliate, partner, employee, independent contractor, director, officer, volunteer, coach, athlete, manager, official or member has a reasonable belief that a minor is being abused, molested or neglected, he/she shall report this belief to authorities, and shall immediately advise a manager, either, Thomas Finks and/or an On-Site Football Director. All such allegations will be turned over to the authorities for investigation.
The High School and/or Pro Sports Experience shall immediately relieve the suspected abuser from his or her duties pending investigation by child protection authorities and/or police.

SAFETY/INJURY CONTROL PROCEDURES
1. A list of emergency telephone numbers for parents of players will be distributed to coaches and staff.
2. In the event of a large-scale emergency or disaster, coaches shall immediately account for all participants and proceed with them to an area of safety. Coaches and trainers shall follow all instructions of local and/or federal agencies, when available. As soon as appropriate, coaches and trainers shall call each participant’s emergency contact number in an effort to reunite the participant with the family as quickly as practicable. In the event of a major injury, coaches shall call 9-1-1 immediately. An injury report should be filled out by the head coach and submitted to PSE offices as soon as possible.
3. Fluid replacement opportunities will be scheduled into practice sessions. Participants will be encouraged to re-hydrate whenever they want. Contractors will emphasize the importance of hydration and sun protection during the camp operations. All coaches and trainers will monitor participants for potential hydration deficiencies.
4. The High School and PSE will implement a zero-tolerance policy for any malicious, violent or unsportsmanlike act at any time during camp activity.
5. Each session, all coaches and counselors will verify proper equipment is being worn by all participants. All activities and drills will be evaluated with safety being the most important criteria. No participant will be allowed on the playing surface without proper equipment.
6. A first aid kit will be available for the head coach and/or On-Site Football Camp Director.
7. All players will be asked to report any minor injury to the On-Site Football Director for the purpose of monitoring the athlete and prevention of aggravation. An injury report should be filled out by the On-Site Football Director submitted to the High School and/or PSE offices within 24 hours. I have read and agree to the above Safety – Injury Control policy.
