Sweepstakes Home / Vote Now for Your Favorite Youth Sports Organization 1. Eligibility: You must be at least 21 years of age and a legal resident of the state of Wisconsin. Employees and directors of the Sponsor, and its subsidiaries, affiliates, distributors, retail dealers, advertising and promotional agencies (collectively the Sweepstakes Entities), and the immediate family members or those living in the same household with such individuals, are not eligible to participate. All federal, state and local laws and regulations apply. 1. Enter on-line, without making a purchase at http://www.surveymonkey.com/s/winsportsequipmentwi. (Limit: one online entry per person). 2. You may also enter by mail, without making a purchase, by sending either the official entry form (downloaded from FORM HERE) or a hand-printed 3 x 5 piece of paper or postcard, in an envelope, including your name, complete address and zip code, day or evening phone number, email address (if you have one) and the full name, contact information and location of the one (1) youth sports organization you would most like to receive the $1,000 in sports equipment in this Sweepstakes to: Pro Sports Experience Win $1,000 in Equipment for Your Youth Sports League Sweepstakes , c/o Pro Sports Experience, LLC, 213 N. Morgan St., #1A, Chicago, IL 60607. Must be postmarked on or before 3/30/12 and received on or before 4/7/12. (See Rule 2, below, for a description of how youth sports organizations will be selected to receive the prize) Limit one (1) Entry per person and per email address for the duration of the Promotion Period. In case of multiple Entries from any one person or email address, only the first Entry received will be accepted. Multiple Entries for any one (1) youth sports league and/or organization are allowed; however, each Entry must be submitted by a different person/email address. Sponsor is not responsible for lost, damaged, illegible, late, misdirected, or postage-due mail, nor for Internet submissions not received due to technical/transmission problems. By entering this Sweepstakes, entrants accept and agree to be bound by these 2. Determination of Winners: For the Grand Prize awarded to a youth sports organization, following the conclusion of the Promotion Period, Sponsor shall tabulate all valid entries received in accordance with these Official Rules. Sponsor may, but is not obligated to, periodically post the then-current results at www.ProSportExperience.com during the Promotion Period. The youth sports organization named on the greatest number of entries will be declared the potential prize winner (subject to verification of eligibility). The final results will be posted on the above Web site on or before April 13, 2012. In the event of a tie, a random drawing between/among the tied youth sports organizations will be conducted by the Sponsor to determine the potential winner. The winning youth sports organization will be notified by phone, email and/or mail. An authorized representative of the youth sports organization will be required to sign and return an Affidavit of Eligibility and Liability/Publicity release form (where permitted) within ten (10) days of issuance as a condition of awarding a prize to the youth sports organization. If any prize notification is returned as undeliverable, or if any authorized youth sports organization representative fails to provide the required documents in a timely manner, or if a potential winning youth sports organization is determined to be ineligible to receive a prize for any reason, the corresponding prize may be forfeited, in the sole discretion of Sponsor. If any prize is forfeited, it may be awarded to an alternate winning youth sports organization based on the manner of selection described above. The entrants who submitted the entries corresponding to the winning youth sports organization will not receive prizes of any kind. 3. Prize/Odds: There is one (1) Grand Prize awarded to a registered youth sports organization located in the state of Wisconsin and located within 140 miles of Kenosha, Wisconsin (53142) and serving youth in any grade from kindergarten through 8th grade. The Grand Prize is a $1,000 gift certificate to a sporting goods supplier which will be awarded to the youth sports organization that receives the most votes during the Promotion Period. Total Approximate Retail Value (ARV) of this prize is $1,000.00 USD. Following verification of eligibility, the winning youth sports organization will receive $1,000 in the form of a gift certificate to a sporting goods store and/or supplier determined by the Sponsor. The winning youth sports organization will not receive any additional money from Sponsor to cover taxes and/or shipping/delivery costs beyond the $1,000 gift certificate. The winning youth sports organization must use the gift certificate or add their own monies to cover taxes, shipping, delivery and any other charges relating to the prize in excess of the $1,000 gift certificate. Sponsor reserves the right to record, photograph and/or videotape all those attending the presentation of the gift certificate to the winning youth sports organization (Attendees) (and such recordings, photographs and/or videotapes shall be owned by Sponsor), and use such recordings, photographs and/or videotapes in any and all media now known or hereafter created throughout the world in perpetuity without compensation to winner or any third party. Winner may be required to send a photo of him/herself/team/organization, to be used for publicity purposes. Grand Prize award is dependent upon full approval and cooperation of applicable youth sports organization. All prizes will be awarded, provided they are properly claimed in accordance with these Official Rules. Prizes are non-transferable and no substitutions will be made. There is no cash value to the Prize and the Prize may not be exchanged for any cash, services or goods. By accepting a prize, the winning youth sports organization agrees that Sponsor and its designees may use (unless prohibited by law), the name of such youth sports organization, location, images, photos, logos, and/or trademarks, for advertising, trade and/or any other purposes in any media now or hereafter known throughout the world in perpetuity, without further compensation, permission or notification. All costs and expenses associated with the acceptance and use of a prize not specified herein as being awarded, including, but not limited to, all federal, state and local taxes, are the sole responsibility of the winning youth sports organization. All prize details are at the sole discretion of the Sponsor. 4.Winner Announcement: The final results will be posted at www.ProSportExperience.com on or about April 13, 2012. 5.General Conditions and Limitations: The entrants who submitted the entries corresponding to the winning youth sports organization will not receive prizes of any kind. Prizes are non-transferable, and no substitutions will be made. All prizes will be awarded, provided they are properly claimed in accordance with these Official Rules. By entering, each Sweepstakes entrant and, by accepting any prize, the winning youth sports organization, agree to hold harmless and indemnify the Sweepstakes Entities and each of their affiliates, directors, officers, employees and agents against any and all liability, damages or causes of action (however named or described), with respect to or arising out of participation by the entrant in this Sweepstakes, and/or the receipt, use, or misuse of the prize by the winning youth sports organization awarded herein. By participating in this Sweepstakes, entrants/winner agree to be bound by these Official Rules and the decisions of the Sponsor. Entrants and winner further agree that by participating, they may from time to time receive promotional offerings from the Sponsor or their marketing partners/affiliates. All disputes concerning this promotion will be governed by and construed in accordance with the laws, rule and regulations of the State of Illinois. Sponsor reserves the right to terminate, suspend or modify the Sweepstakes if, at its sole discretion, it determines that for any reason the Sweepstakes is not capable of running as originally planned, or fraud, technical failures, printing, mailing or distribution errors, or any other unanticipated causes or occurrences have compromised the fairness or integrity of the Sweepstakes. In the event the Sweepstakes is terminated prior to the stated end date, Sponsor reserves the right to award the Grand Prize in a random drawing from among all eligible entries received as of the date of termination. TO THE MAXIMUM EXTENT PERMITTED BY LAW, THE SWEEPSTAKES ENTITIES DISCLAIM ALL WARRANTIES AND CONDITIONS REGARDING THE PRIZE, WHETHER EXPRESS OR IMPLIED, INCLUDING, BUT NOT LIMITED TO, THE IMPLIED WARRANTIES OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR OR GENERAL USE OR PURPOSE. 6.Winner List: Winner will be announced on the website of the Sponsor.
